Home & Community Habilitation

Home & Community Habilitation focuses on helping individuals develop essential life skills at home and in the community. Our dedicated staff works one‑on‑one to encourage growth, independence, and meaningful participation in daily routines.

Supports include:

  • Daily living skills (cooking, cleaning, hygiene, organization)

  • Community participation (shopping, events, volunteering, activities)

  • Social and communication development

  • Safety awareness (emergency readiness, personal boundaries, community safety)

  • Basic money management (budgeting, spending, banking)

  • Transportation training (bus access, route planning, walking safety)

  • Wellness and healthy habit building

Companion Services

Companion Services provide individuals with support and supervision to encourage social interaction, ensure personal safety, and assist with everyday activities both at home and in the community.

Supports include:

  • Assistance with attending appointments

  • Social and recreational activities

  • Community outings (parks, dining, events, shopping)

  • Emotional support and companionship

  • Encouragement in completing daily tasks

  • Monitoring well‑being and safety

Homemaker / Chore Services

Homemaker and Chore Services ensure that individuals live in a clean, organized, and safe home environment. This service supports daily household tasks that may be difficult to manage alone.

Supports include:

  • Light housekeeping (dusting, sweeping, mopping)

  • Meal preparation and cooking assistance

  • Laundry and linen care

  • Dishwashing and kitchen cleaning

  • Trash removal

  • Household organization

  • Grocery shopping and essential errands

Respite Care – Out of Home

Relief for Caregivers • Quality Care for Individuals

Respite Care provides temporary, out‑of‑home support for individuals, giving caregivers time to rest or attend to personal needs. During respite, individuals receive compassionate supervision and participate in structured, enjoyable activities.

Supports include:

  • Safe, comfortable short‑term care

  • Engaging activities and social opportunities
